Residents of Angeles City, especially senior citizens, are now entitled to free cataract surgery starting this month, thanks to a joint project of Angeles City Mayor Carmelo Lazatin Jr. and his brother, Pampanga Rep. Carmelo Lazatin II.

Under Memorandum 1445, city residents may apply for the operation provided they are eligible.

Forms shall be provided in the city’s six Rural Health Units and 28 sub-health centers from October 25 to November 5, according to the memo.

Residents who are interested in the program are requied to fill out the form and submit the necessary requirements to the staff of the RHUs or sub-health centers.

Mandatory documents include valid IDs with city address; Certificate of Barangay Indigency with the same address indicated in the valid IDs; and social case study or endorsement from the City Social Welfare and Development Office.

“Aslag ing pag-asa para kareng Angeleños a miki kapanandaman keng mata kalupa na ning katarata,” Lazatin said.
